Physical Therapist (PT) – Home Health

Position Details

  • Schedule: Monday – Friday, 8:00 AM – 5:00 PM
  • Weekend Requirement: None
  • On-Call Requirement: None
  • Productivity Benchmark: 28 units/points per week
  • Mileage Reimbursement: $0.54 per mile
  • Coverage Area: Raleigh, Logan, and Wyoming Counties, WV
  • License Requirement: Active West Virginia Physical Therapist License or Compact State License

Required Qualifications

  • Home Health experience required
  • OASIS documentation experience required
  • HCHB (Homecare Homebase) experience preferred
  • Strong clinical assessment and care planning skills
  • Excellent communication and documentation abilities
  • Ability to work independently in a field-based environment

Position Summary


The Physical Therapist (PT) provides skilled physical therapy services to patients in the home health setting. The PT is responsible for evaluating patients, developing individualized treatment plans, implementing therapeutic interventions, and collaborating with healthcare providers to improve patient mobility, independence, and overall quality of life.

Key Responsibilities

  • Perform comprehensive physical therapy evaluations and assessments based on physician referrals and orders.
  • Review patient diagnoses, medical history, precautions, and contraindications to establish safe and effective treatment plans.
  • Develop measurable goals and individualized care plans tailored to each patient's functional needs.
  • Provide skilled therapy interventions to improve strength, mobility, balance, endurance, and independence.
  • Monitor patient progress and modify treatment plans as necessary to achieve optimal outcomes.
  • Collaborate with physicians, interdisciplinary team members, patients, and caregivers regarding treatment goals, progress, and discharge planning.
  • Recommend and assist with obtaining adaptive equipment and assistive devices to enhance patient safety and function.
  • Maintain accurate and timely clinical, treatment, and billing documentation in compliance with company standards and regulatory requirements.
  • Adhere to HIPAA regulations, company policies, professional practice standards, and all applicable state and federal laws.
  • Supervise Physical Therapist Assistants (PTAs) and therapy aides in accordance with state practice regulations.
  • Participate in staff meetings, continuing education programs, and professional development activities to maintain and enhance clinical competency.

The ideal candidate is an experienced Home Health Physical Therapist with strong OASIS knowledge and a commitment to delivering exceptional patient-centered care. Experience with HCHB is preferred. The candidate should possess excellent clinical judgment, strong organizational skills, and the ability to work independently while managing a diverse patient caseload.
